Booking Safety Acknowledgment

Before every booking on the Firearm Mentorship Network platform (the "Platform"), the Participant (or Guardian, for a Minor) must confirm each statement below. By checking the box, I acknowledge and agree:

1. Session-Specific Review

☐ I have read the Session listing, including its location, prerequisites, required equipment, total cost, age eligibility, Provider waiver (if any), and cancellation terms, and I meet the stated prerequisites.

2. Independent Provider

☐ I understand the Session is provided by an independent Instructor or Mentor, not by FMN; that FMN does not supervise or attend the Session; and that FMN's limited review does not verify every credential or guarantee safety. I have had the opportunity to ask the Provider about qualifications, insurance, and range arrangements.

3. Firearm Safety Rules

☐ I will follow the universal firearm safety rules at all times: treat every firearm as loaded; keep the muzzle in a safe direction; keep my finger off the trigger until ready to fire; and know my target and what is beyond it.

☐ I will follow every instruction of the Provider and range safety officer immediately, including "cease fire."

4. Fitness and Sobriety

☐ I will arrive sober and will not consume alcohol, cannabis, or impairing drugs or medications before or during the Session. I will privately inform the Provider of any physical or medical condition that may affect my safety.

5. Equipment and Transport

☐ I will bring only firearms and ammunition the Provider and range permit, and I will transport them unloaded, cased, and in compliance with the laws of every jurisdiction I travel through. I will not bring loaded firearms into classroom areas or handle firearms until instructed.

☐ I will wear eye and ear protection during live fire and whenever required.

6. Legal Eligibility

☐ For any live-fire or hands-on Session, I am not prohibited by federal, state, or local law from possessing firearms or ammunition, and I am not subject to a protective order that restricts firearm possession.

7. Range Rules

☐ I will follow all range rules and sign any range waiver required by the facility.

8. Risks and Release

☐ I have read and accept the Assumption of Risk and the Participant Waiver and Release of Liability, which apply to this Session.

9. Right to Stop; Removal

☐ I understand I may stop at any time, and that the Provider may remove me for unsafe conduct, with refund eligibility governed by the Cancellation Policy.

10. Emergencies and Reporting

☐ I will call 911 in an emergency and will report safety incidents and concerns to safety@firearmmentorshipnetwork.com.

11. Minors (Guardians only)

☐ I am the Minor's parent or legal guardian; I have accepted the Parent/Guardian Consent and Minor Participation Agreement; this Session is designated as a Youth Class suitable for my Minor's age; and I will attend or remain responsible as that agreement requires.

☐ I confirm all of the above for this booking.

Accepted electronically on the Platform. FMN records the name of the accepting user, the date and time, the IP address, and the document version, and the booking for each acceptance.
