Parent/Guardian Consent and Minor Participation Agreement

READ CAREFULLY. THIS AGREEMENT AFFECTS YOUR LEGAL RIGHTS AND YOUR RESPONSIBILITIES FOR YOUR CHILD.

1. Purpose

This Agreement governs participation by a person under 18 ("Minor") in firearms training on the Firearm Mentorship Network platform (the "Platform"). Minors may participate only in Sessions designated by the Provider as Youth Classes, and only through the account of the Minor's parent or legal guardian ("Guardian," "I," "me"). This Agreement is in addition to the Terms of Service, Privacy Policy, Safety Policy, Booking Agreement, Cancellation Policy, Booking Safety Acknowledgment, Participant Waiver and Release of Liability, and Assumption of Risk, all of which I accept for myself.

2. Guardian Representations

I represent that:

1. I am at least 18 years old and am the Minor's parent or court-appointed legal guardian with legal authority to consent to the Minor's participation and medical treatment;
2. I am not prohibited by federal, state, or local law from possessing firearms or ammunition;
3. If another parent or guardian shares legal custody, I have authority to consent on my own, or I have obtained any required consent; and
4. All information I provide about the Minor is accurate.

3. How Minor Participation Works

3.1 Guardian account only. I will create and manage the Minor's sub-profile from my own account. The Minor will have no login, no messaging access, and no public profile.

3.2 Guardian controls everything. I will make all bookings, communications, cancellations, and reviews for the Minor. Providers will communicate only with me.

3.3 Minimum information. I will provide only the information needed for safe participation: the Minor's first name and last initial, age or date of birth, experience level, emergency contact, and any safety-relevant notes. I will not upload photos of the Minor to the Platform.

3.4 Youth Classes only. I will book only Sessions that the Provider has designated as Youth Classes and that are appropriate for the Minor's age, size, maturity, and experience. The Minor must meet the Session's minimum age as set by the Provider or required by law.

4. Attendance and Responsibility

4.1 Required attendance. I will personally attend every Session with the Minor and remain on the premises for its entire duration.

4.2 Supervision. I understand that the Provider is responsible for instruction and range safety during the Session, and that I remain responsible for the Minor's conduct, wellbeing, and supervision at all other times, including arrival, breaks, and departure.

4.3 Pick-up. I will not leave the Minor at any training location without me or the Designated Adult present.

5. Consent to Participation, Including Handgun Instruction

5.1 General consent. I consent to the Minor's participation in the Youth Classes I book, including classroom instruction, dry practice, and supervised live fire as described in the Session listing.

5.2 Written consent for handgun instruction. For purposes of 18 U.S.C. § 922(x)(3) and any similar state law, I give my prior written consent for the Minor to temporarily possess a handgun, under the direct supervision of the Provider, solely during and in the course of instruction in the safe and lawful use of a handgun (and target practice that is part of that instruction) at the Youth Classes I book. I will print or save a signed copy of this consent, and I will ensure the copy is in the Minor's or my possession during any Session involving a handgun, as the law requires. I understand that any handgun must be transported to and from the Session unloaded and in a locked container, and that I am responsible for the lawful transport of any firearm I bring.

5.3 Withdrawal of consent. I may withdraw consent at any time by cancelling the booking or removing the Minor's sub-profile.

6. Minor's Safety Rules

I will review the FMN Safety Policy and the universal firearm safety rules with the Minor before each Session, and I will ensure the Minor understands that they must follow every instruction immediately. I understand the Provider may remove the Minor (and me) from a Session for unsafe conduct.

7. Youth Protection Standards

I understand that Providers must follow FMN youth-protection standards, including: communicating only with me; never being alone with the Minor out of view of me or another adult; never transporting the Minor; and never photographing or posting images of the Minor without my written consent. I will report any violation to safety@firearmmentorshipnetwork.com and, where appropriate, to law enforcement.

8. Photographs

☐ (Optional) I consent to the Provider photographing the Minor during the Session for the Provider's private records or to share with me. I do not consent to public posting unless I separately consent in writing. (Leave unchecked to refuse.)

9. Medical Authorization

In an emergency, if I cannot be reached immediately, I authorize the Provider, range personnel, and emergency responders to obtain and consent to emergency medical treatment for the Minor that a licensed medical professional considers necessary. I am responsible for all costs. I will disclose privately to the Provider any condition, allergy, or medication relevant to the Minor's safety.

10. Assumption of Risk

I understand that firearms training involves inherent risks that cannot be eliminated, including serious bodily injury, permanent disability, hearing and eye damage, lead exposure, and death, as described in the Assumption of Risk, and that Minors may face greater risk due to their size, strength, maturity, and inexperience. I knowingly and voluntarily assume these risks on my own behalf and, to the extent permitted by law, on behalf of the Minor.

11. Release, Waiver, and Indemnification

11.1 Release of my own claims. To the fullest extent permitted by law, I release, waive, and covenant not to sue the Released Parties (as defined in the Participant Waiver and Release of Liability) for any claims I may have, including claims for the Minor's medical expenses and loss of services, arising out of the Minor's participation, including claims arising from the Released Parties' ordinary negligence.

11.2 Release on behalf of the Minor. To the fullest extent permitted by the law of the applicable state, I also release such claims on behalf of the Minor. I understand that some states do not permit a parent to waive a minor's claims, and that in those states this Section 11.2 may not be enforceable.

11.3 Parental indemnification. I agree to indemnify, defend, and hold harmless the FMN Parties from any claim brought by or on behalf of the Minor (including after the Minor reaches the age of majority) arising out of the Minor's participation, and from any claim arising out of my or the Minor's breach of this Agreement, except to the extent caused by the gross negligence or intentional misconduct of an FMN Party.

11.4 Exclusions. Nothing in this Agreement releases any person from liability for gross negligence, wantonness, recklessness, intentional misconduct, or abuse.

12. Privacy of the Minor's Information

FMN will handle the Minor's information as described in Section 7 of the Privacy Policy: collected only from me, minimized, shared only with the Provider of the booked Youth Class, never used for advertising or sponsor analytics, never publicly displayed, and deleted on my request or within 12 months after the Minor's last Session, subject to legal and safety retention needs. I may review, correct, or delete the Minor's information by emailing privacy@firearmmentorshipnetwork.com.

13. Term

This Agreement applies to all Youth Classes I book for the Minor until I withdraw it or the Minor turns 18. FMN may require me to re-accept this Agreement periodically or for a specific booking. When the Minor turns 18, the Minor's sub-profile will be closed; the former Minor may create their own account.

14. Governing Law

This Agreement is governed by the laws of the State of Alabama, subject to the dispute-resolution provisions of the Terms of Service.

Signature and Acceptance

I HAVE READ THIS AGREEMENT, UNDERSTAND THAT IT AFFECTS MY LEGAL RIGHTS AND THE MINOR'S, AND SIGN IT VOLUNTARILY.

Accepted electronically on the Platform. FMN records the name of the accepting user, the date and time, the IP address, and the document version, and the booking for each acceptance. Print or save a copy of this accepted consent for the purposes described in Section 5.2.
