Training Request Safety Acknowledgment

Before posting or sending a Training Request on the Firearm Mentorship Network platform (the "Platform"), you must read and accept this Safety Acknowledgment. By checking the box, you (and, if you are a Guardian, you on behalf of yourself and your Minor) acknowledge and agree to each of the following:

1. FMN Is a Connection Service Only

I understand that FMN only helps me find and communicate with independent Instructors and Mentors ("Providers"). FMN does not provide, supervise, or attend Training; does not employ Providers; and is not a party to any agreement I make with a Provider.

2. Limited Verification

I understand that FMN's review of Providers is limited, that FMN does not verify every credential, certification, insurance policy, reference, or claim, and that background-check status, where shown, is optional, may be out of date, and is not a guarantee of safety. I am responsible for evaluating and choosing any Provider, including by verifying credentials and asking about insurance, range arrangements, and safety practices.

3. Inherent Risks

I understand that firearms training involves inherent risks that cannot be eliminated, including serious bodily injury, permanent disability, hearing damage, eye injury, lead exposure, and death, caused by my own actions, the actions of others, equipment malfunction, or range conditions.

4. Safety Commitments

I will follow the FMN Safety Policy, the universal firearm safety rules, and all instructions of the Provider and range staff; I will not handle firearms while impaired; and I will stop and ask whenever I am unsure.

5. Meeting Safely

I will keep communications on the Platform where practical, meet at reputable ranges or public facilities, tell someone where I will be, and leave any situation that feels unsafe. I will never pay by wire transfer, gift card, or cryptocurrency at a Provider's request.

6. Lawful Purpose and Eligibility

I am seeking Training for lawful purposes only. I am not prohibited by law from possessing firearms or ammunition (for live-fire Training), and I will not use the Platform to buy, sell, or transfer firearms or ammunition.

7. Additional Documents

I understand that if I book Training, I will be required to accept the Booking Agreement, Booking Safety Acknowledgment, Participant Waiver and Release of Liability, and Assumption of Risk, and may be required to sign the Provider's or range's own waiver.

8. Emergencies and Reports

In an emergency I will call 911. I will report safety concerns to safety@firearmmentorshipnetwork.com.

Accepted electronically on the Platform. FMN records the name of the accepting user, the date and time, the IP address, and the document version for each acceptance.
